Insurance
Dr. Joseph Militello is a participating provider with most major health insurance plans, including HMO's, PPO's, Medicare, and Workman's Compensation. We have provided a listing of our current insurances plans below. The Eye Center is always adding new insurances. If you don't see your plan listed, please call our office at 352-683-4500 to see if we are a participator with your insurance.
It is the policy of The Eye Center to collect co-payments and deductibles when at the time of service per the specifics of your insurance plan. Payments may be made by cash, check, Mastercard or Visa. For procedures not covered by insurance, our Billing Specialist will work with you to create a payment plan.
The Eye Center will file your primary and secondary insurances for you. We ask that you always keep our office updated with any name, address, or insurance changes so we can assist you with your claims.
